Deadline May 10th - Developmental Screening

Dear EEC Families! (if you already completed in 2023 please disregard)

We are required by the state to support families and children receive early intervention services at the time of need.


We need you to complete the ASQ - Ages & Stages Questionnaire. Upon enrollment you received a consent form to complete this. However, now that it is up to parents to complete and for us to monitor and if any interventions are recommended to help support families in that process. Please complete the questionnaire appropriate for your child’s age by May 10th!!!
